35b4bfbda5 scsi: libsas: align sata_device's rps_resp on a cacheline
commit c2e8fbf908afd81ad502b567a6639598f92c9b9d upstream.

The rps_resp buffer in ata_device is a DMA target, but it isn't
explicitly cacheline aligned. Due to this, adjacent fields can be
overwritten with stale data from memory on non-coherent architectures.
As a result, the kernel is sometimes unable to communicate with an SATA
device behind a SAS expander.

Fix this by ensuring that the rps_resp buffer is cacheline aligned.

This issue is similar to that fixed by Commit 84bda12af3 ("libata:
align ap->sector_buf") and Commit 4ee34ea3a1 ("libata: Align
ata_device's id on a cacheline").

fc866b296a scsi: Add STARGET_CREATED_REMOVE state to scsi_target_state

The addition of the STARGET_REMOVE state had the side effect of
introducing a race condition that can cause a crash.

scsi_target_reap_ref_release() checks the starget->state to
see if it still in STARGET_CREATED, and if so, skips calling
transport_remove_device() and device_del(), because the starget->state
is only set to STARGET_RUNNING after scsi_target_add() has called
device_add() and transport_add_device().

However, if an rport loss occurs while a target is being scanned,
it can happen that scsi_remove_target() will be called while the
starget is still in the STARGET_CREATED state.  In this case, the
starget->state will be set to STARGET_REMOVE, and as a result,
scsi_target_reap_ref_release() will take the wrong path.  The end
result is a panic:

There's a rather long standing regression from the commit "libiscsi:
Reduce locking contention in fast path"

Depending on iSCSI target behavior, it's possible to hit the case in
iscsi_complete_task where the task is still on a pending list
(!list_empty(&task->running)).  When that happens the task is removed
from the list while holding the session back_lock, but other task list
modification occur under the frwd_lock.  That leads to linked list
corruption and eventually a panicked system.

Rather than back out the session lock split entirely, in order to try
and keep some of the performance gains this patch adds another lock to
maintain the task lists integrity.

Major enterprise supported kernels have been backing out the lock split
for while now, thanks to the efforts at IBM where a lab setup has the
most reliable reproducer I've seen on this issue.  This patch has been
tested there successfully.

70e7588923 scsi: add option to override the command timeout
Some platforms might be very slow and command completion may take
much longer than default scsi command timeouts.
SCSI Read/Write command timeout can be changed by blk_queue_rq_timeout()
but there is no option to override timeout for rest of the scsi commands.
This change allows the LLD to override the default scsi command timeout
for all scsi commands (except read/write).

4b9bc86d5a fcoe: convert to kworker
The driver creates its own per-CPU threads which are updated based on
CPU hotplug events. It is also possible to use kworkers and remove some
of the kthread infrastrucure.

The code checked ->thread to decide if there is an active per-CPU
thread. By using the kworker infrastructure this is no longer
possible (or required). The thread pointer is saved in `kthread' instead
of `thread' so anything trying to use thread is caught by the
compiler. Currently only the bnx2fc driver is using struct fcoe_percpu_s
and the kthread member.

After a CPU went offline, we may still enqueue items on the "offline"
CPU. This isn't much of a problem. The work will be done on a random
CPU. The allocated crc_eof_page page won't be cleaned up. It is probably
expected that the CPU comes up at some point so it should not be a
problem. The crc_eof_page memory is released of course once the module
is removed.

This patch was only compile-tested due to -ENODEV.

f05795d3d7 scsi: Add intermediate STARGET_REMOVE state to scsi_target_state
Add intermediate STARGET_REMOVE state to scsi_target_state to avoid
running into the BUG_ON() in scsi_target_reap(). The STARGET_REMOVE
state is only valid in the path from scsi_remove_target() to
scsi_target_destroy() indicating this target is going to be removed.

This re-fixes the problem introduced in commits bc3f02a795 ("[SCSI]
scsi_remove_target: fix softlockup regression on hot remove") and
4099819356 ("scsi: restart list search after unlock in
scsi_remove_target") in a more comprehensive way.

1d64508810 scsi: disable automatic target scan
On larger installations it is useful to disable automatic LUN scanning,
and only add the required LUNs via udev rules.  This can speed up bootup
dramatically.

This patch introduces a new scan module parameter value 'manual', which
works like 'none', but can be overridden by setting the 'rescan' value
from scsi_scan_target to 'SCSI_SCAN_MANUAL'.  And it updates all
relevant callers to set the 'rescan' value to 'SCSI_SCAN_MANUAL' if
invoked via the 'scan' option in sysfs.

5ddfe0858e scsi: Do not attach VPD to devices that don't support it
The patch "scsi: rescan VPD attributes" introduced a regression in which
devices that don't support VPD were being scanned for VPD attributes
anyway.  This could cause issues for some devices and should be avoided
so the check for scsi_level has been moved out of scsi_add_lun and into
scsi_attach_vpd so that all callers will not scan VPD for devices that
don't support it.

A bug in the gcc-6.0 prerelease version caused at least one
driver (lpfc) to have excessive stack usage when dealing with
wwn data, on the ARM architecture.

lpfc_scsi.c: In function 'lpfc_find_next_oas_lun':
lpfc_scsi.c:117:1: warning: the frame size of 1152 bytes is larger than 1024 bytes [-Wframe-larger-than=]

I have reported this as a gcc regression in
https://gcc.gnu.org/bugzilla/show_bug.cgi?id=70232

However, using a better implementation of wwn_to_u64() not only
helps with the particular gcc problem but also leads to better
object code for any version or architecture.

The kernel already provides get_unaligned_be64() and
put_unaligned_be64() helper functions that provide an
optimized implementation with the desired semantics.

The lpfc_find_next_oas_lun() function in the example that
grew from 1146 bytes to 5144 bytes when moving from gcc-5.3
to gcc-6.0 is now 804 bytes, as the optimized
get_unaligned_be64() load can be done in three instructions.
The stack usage is now down to 28 bytes from 128 bytes with
gcc-5.3 before.

d3d328919f scsi_dh: add 'rescan' callback
If a device needs to be rescanned the device_handler might need
to be rechecked, too.
So add a 'rescan' callback to the device handler and call it
upon scsi_rescan_device(). The rescan callback will be invoked
from the Unit Attention handling of ASC/ASCQ 3F 03
(INQUIRY DATA HAS CHANGED).

3846470a1b scsi: Export function scsi_scan.c:sanitize_inquiry_string
The hpsa driver uses this function to cleanup inquiry data. Our new pqi
driver will also use this function. This function was copied into both
drivers.

This patch exports sanitize_inquiry_string so the hpsa and the pqi
drivers can use this function directly.
