Jishnu Prakash d09bc698d8 thermal: qpnp-adc-tm: Update adc_tm register offsets for PMIC5 ...

Few adc_tm registers have different offsets on PMIC5 compared
to PMIC4. Update logic to account for PMIC5 registers.
