076ff2f0b8 exofs: use bio_clone_fast in _write_mirror
The mirroring code never changes the bio data or biovecs.  This means
we can reuse the biovec allocation easily instead of duplicating it.

On the quest to remove all VLAs from the kernel[1] this adjusts several
cases where allocation is made after an array of structures that points
back into the allocation.  The allocations are changed to perform
explicit calculations instead of using a Variable Length Array in a
structure.

Additionally, this lets Clang compile this code now, since Clang does
not support VLAIS[2].

6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

 "Currently, hardened usercopy performs dynamic bounds checking on slab
  cache objects. This is good, but still leaves a lot of kernel memory
  available to be copied to/from userspace in the face of bugs.

  To further restrict what memory is available for copying, this creates
  a way to whitelist specific areas of a given slab cache object for
  copying to/from userspace, allowing much finer granularity of access
  control.

  Slab caches that are never exposed to userspace can declare no
  whitelist for their objects, thereby keeping them unavailable to
  userspace via dynamic copy operations. (Note, an implicit form of
  whitelisting is the use of constant sizes in usercopy operations and
  get_user()/put_user(); these bypass all hardened usercopy checks since
  these sizes cannot change at runtime.)

  This new check is WARN-by-default, so any mistakes can be found over
  the next several releases without breaking anyone's system.

  The series has roughly the following sections:
   - remove %p and improve reporting with offset
   - prepare infrastructure and whitelist kmalloc
   - update VFS subsystem with whitelists
   - update SCSI subsystem with whitelists
   - update network subsystem with whitelists
   - update process memory with whitelists

In support of usercopy hardening, this patch defines a region in the
exofs_inode_cache slab cache in which userspace copy operations are
allowed.

This region is known as the slab cache's usercopy region. Slab caches
can now check that each dynamically sized copy operation involving
cache-managed memory falls entirely within the slab's usercopy region.

This patch is modified from Brad Spengler/PaX Team's PAX_USERCOPY
whitelisting code in the last public patch of grsecurity/PaX based on my
understanding of the code. Changes or omissions from the original code are
mine and don't reflect the original grsecurity/PaX code.

74d46992e0 block: replace bi_bdev with a gendisk pointer and partitions index
This way we don't need a block_device structure to submit I/O.  The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open.  Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).

For the actual I/O path all that we need is the gendisk, which exists
once per block device.  But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.

Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

3b49c9a1e9 fs: convert a pile of fsync routines to errseq_t based reporting
This patch converts most of the in-kernel filesystems that do writeback
out of the pagecache to report errors using the errseq_t-based
infrastructure that was recently added. This allows them to report
errors once for each open file description.

Most filesystems have a fairly straightforward fsync operation. They
call filemap_write_and_wait_range to write back all of the data and
wait on it, and then (sometimes) sync out the metadata.

For those filesystems this is a straightforward conversion from calling
filemap_write_and_wait_range in their fsync operation to calling
file_write_and_wait_range.

2b69c8280c mm: drop "wait" parameter from write_one_page()
The callers all set it to 1.

Also, make it clear that this function will not set any sort of AS_*
error, and that the caller must do so if necessary.  No existing caller
uses this on normal files, so none of them need it.

Also, add __must_check here since, in general, the callers need to handle
an error here in some fashion.

c718a97514 fs: semove set but not checked AOP_FLAG_UNINTERRUPTIBLE flag
Commit afddba49d1 ("fs: introduce write_begin, write_end, and
perform_write aops") introduced AOP_FLAG_UNINTERRUPTIBLE flag which was
checked in pagecache_write_begin(), but that check was removed by
4e02ed4b4a ("fs: remove prepare_write/commit_write").

Between these two commits, commit d9414774dc ("cifs: Convert cifs to
new aops.") added a check in cifs_write_begin(), but that check was soon
removed by commit a98ee8c1c7 ("[CIFS] fix regression in
cifs_write_begin/cifs_write_end").

Therefore, AOP_FLAG_UNINTERRUPTIBLE flag is checked nowhere.  Let's
remove this flag.  This patch has no functionality changes.

078cd8279e fs: Replace CURRENT_TIME with current_time() for inode timestamps
CURRENT_TIME macro is not appropriate for filesystems as it
doesn't use the right granularity for filesystem timestamps.
Use current_time() instead.

CURRENT_TIME is also not y2038 safe.

This is also in preparation for the patch that transitions
vfs timestamps to use 64 bit time and hence make them
y2038 safe. As part of the effort current_time() will be
extended to do range checks. Hence, it is necessary for all
file system timestamps to use current_time(). Also,
current_time() will be transitioned along with vfs to be
y2038 safe.

Note that whenever a single call to current_time() is used
to change timestamps in different inodes, it is because they
share the same time granularity.

31051c85b5 fs: Give dentry to inode_change_ok() instead of inode
inode_change_ok() will be resposible for clearing capabilities and IMA
extended attributes and as such will need dentry. Give it as an argument
to inode_change_ok() instead of an inode. Also rename inode_change_ok()
to setattr_prepare() to better relect that it does also some
modifications in addition to checks.

95fe6c1a20 block, fs, mm, drivers: use bio set/get op accessors
This patch converts the simple bi_rw use cases in the block,
drivers, mm and fs code to set/get the bio operation using
bio_set_op_attrs/bio_op

These should be simple one or two liner cases, so I just did them
in one patch. The next patches handle the more complicated
cases in a module per patch.

be5b82dbfe make ext2_get_page() and friends work without external serialization
Right now ext2_get_page() (and its analogues in a bunch of other filesystems)
relies upon the directory being locked - the way it sets and tests Checked and
Error bits would be racy without that.  Switch to a slightly different scheme,
_not_ setting Checked in case of failure.  That way the logics becomes
	if Checked => OK
	else if Error => fail
	else if !validate => fail
	else => OK
with validation setting Checked or Error on success and failure resp. and
returning which one had happened.  Equivalent to the current logics, but unlike
the current logics not sensitive to the order of set_bit, test_bit getting
reordered by CPU, etc.

09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized.  And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE.  And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special.  They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} -> PAGE_{SIZE,SHIFT,MASK,ALIGN};

 - page_cache_get() -> get_page();

 - page_cache_release() -> put_page();

This patch contains automated changes generated with coccinelle using
script below.  For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach.  I'll
fix them manually in a separate patch.  Comments and documentation also
will be addressed with the separate patch.

userspace as __GFP_ACCOUNT/SLAB_ACCOUNT, which makes them accounted to

 - vm_area_struct and vm_region (nommu)

 - dentry and external_name
 - inode for all filesystems. This is the most tedious part, because
   most filesystems overwrite the alloc_inode method.

The list is far from complete, so feel free to add more objects.
Nevertheless, it should be close to "account everything" approach and
keep most workloads within bounds.  Malevolent users will be able to
breach the limit, but this was possible even with the former "account
everything" approach (simply because it did not account everything in
fact).

3066a9670b osd fs: __r4w_get_page rely on PageUptodate for uptodate
Commit 42cb14b110 ("mm: migrate dirty page without
clear_page_dirty_for_io etc") simplified the migration of a PageDirty
pagecache page: one stat needs moving from zone to zone and that's about
all.

It's convenient and safest for it to shift the PageDirty bit from old
page to new, just before updating the zone stats: before copying data
and marking the new PageUptodate.  This is all done while both pages are
isolated and locked, just as before; and just as before, there's a
moment when the new page is visible in the radix_tree, but not yet
PageUptodate.  What's new is that it may now be briefly visible as
PageDirty before it is PageUptodate.

When I scoured the tree to see if this could cause a problem anywhere,
the only places I found were in two similar functions __r4w_get_page():
which look up a page with find_get_page() (not using page lock), then
claim it's uptodate if it's PageDirty or PageWriteback or PageUptodate.

I'm not sure whether that was right before, but now it might be wrong
(on rare occasions): only claim the page is uptodate if PageUptodate.
Or perhaps the page in question could never be migratable anyway?

21fc61c73c don't put symlink bodies in pagecache into highmem
kmap() in page_follow_link_light() needed to go - allowing to hold
an arbitrary number of kmaps for long is a great way to deadlocking
the system.

new helper (inode_nohighmem(inode)) needs to be used for pagecache
symlinks inodes; done for all in-tree cases.  page_follow_link_light()
instrumented to yell about anything missed.
