84e16102a1 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The Coccinelle script used for this was:

b6399c388a treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

5ed9709d26 powerpc/xive: Fix refcount leak in xive_get_max_prio

of_find_node_by_path() returns a node pointer with
refcount incremented, we should use of_node_put() on it when done.
Add missing of_node_put() to avoid refcount leak.

By default old pre-3.0 Freescale PCIe controllers reports invalid PCI Class
Code 0x0b20 for PCIe Root Port. It can be seen by lspci -b output on P2020
board which has this pre-3.0 controller:

  $ lspci -bvnn
  00:00.0 Power PC [0b20]: Freescale Semiconductor Inc P2020E [1957:0070] (rev 21)
          !!! Invalid class 0b20 for header type 01
          Capabilities: [4c] Express Root Port (Slot-), MSI 00

Fix this issue by programming correct PCI Class Code 0x0604 for PCIe Root
Port to the Freescale specific PCIe register 0x474.

With this change lspci -b output is:

  $ lspci -bvnn
  00:00.0 PCI bridge [0604]: Freescale Semiconductor Inc P2020E [1957:0070] (rev 21) (prog-if 00 [Normal decode])
          Capabilities: [4c] Express Root Port (Slot-), MSI 00

Without any "Invalid class" error. So class code was properly reflected
into standard (read-only) PCI register 0x08.

Same fix is already implemented in U-Boot pcie_fsl.c driver in commit:
d18d06ac35

Fix activated by U-Boot stay active also after booting Linux kernel.
But boards which use older U-Boot version without that fix are affected and
still require this fix.

So implement this class code fix also in kernel fsl_pci.c driver.

85fb0f4a2d powerpc/dcr: Use cmplwi instead of 3-argument cmpli

In dcr-low.S we use cmpli with three arguments, instead of four
arguments as defined in the ISA:

	cmpli	cr0,r3,1024

This appears to be a PPC440-ism, looking at the "PPC440x5 CPU Core
User’s Manual" it shows cmpli having no L field, but implied to be 0 due
to the core being 32-bit. It mentions that the ISA defines four
arguments and recommends using cmplwi.

It also corresponds to the old POWER instruction set, which had no L
field there, a reserved bit instead.

dcr-low.S is only built 32-bit, because it is only built when
DCR_NATIVE=y, which is only selected by 40x and 44x. Looking at the
generated code (with gcc/gas) we see cmplwi as expected.

Although gas is happy with the 3-argument version when building for
32-bit, the LLVM assembler is not and errors out with:

  arch/powerpc/sysdev/dcr-low.S:27:10: error: invalid operand for instruction
   cmpli 0,%r3,1024; ...
           ^

Switch to the cmplwi extended opcode, which avoids any confusion when
reading the ISA, fixes the issue with the LLVM assembler, and also means
the code could be built 64-bit in future (though that's very unlikely).

ad5399b948 powerpc/xive: Clear the page tables for the ESB IO mapping
[ Upstream commit a101950fcb78b0ba20cd487be6627dea58d55c2b ]

Commit 1ca3dec2b2df ("powerpc/xive: Prevent page fault issues in the
machine crash handler") fixed an issue in the FW assisted dump of
machines using hash MMU and the XIVE interrupt mode under the POWER
hypervisor. It forced the mapping of the ESB page of interrupts being
mapped in the Linux IRQ number space to make sure the 'crash kexec'
sequence worked during such an event. But it didn't handle the
un-mapping.

This mapping is now blocking the removal of a passthrough IO adapter
under the POWER hypervisor because it expects the guest OS to have
cleared all page table entries related to the adapter. If some are
still present, the RTAS call which isolates the PCI slot returns error
9001 "valid outstanding translations".

Remove these mapping in the IRQ data cleanup routine.

Under KVM, this cleanup is not required because the ESB pages for the
adapter interrupts are un-mapped from the guest by the hypervisor in
the KVM XIVE native device. This is now redundant but it's harmless.

46d7381f78 powerpc/xive: Use XIVE_BAD_IRQ instead of zero to catch non configured IPIs
commit b1a504a6500df50e83b701b7946b34fce27ad8a3 upstream.

When a CPU is brought up, an IPI number is allocated and recorded
under the XIVE CPU structure. Invalid IPI numbers are tracked with
interrupt number 0x0.

On the PowerNV platform, the interrupt number space starts at 0x10 and
this works fine. However, on the sPAPR platform, it is possible to
allocate the interrupt number 0x0 and this raises an issue when CPU 0
is unplugged. The XIVE spapr driver tracks allocated interrupt numbers
in a bitmask and it is not correctly updated when interrupt number 0x0
is freed. It stays allocated and it is then impossible to reallocate.

Fix by using the XIVE_BAD_IRQ value instead of zero on both platforms.

3a4c1a8952 powerpc/xive: Skip ioremap() of ESB pages for LSI interrupts

The PCI INTx interrupts and other LSI interrupts are handled differently
under a sPAPR platform. When the interrupt source characteristics are
queried, the hypervisor returns an H_INT_ESB flag to inform the OS
that it should be using the H_INT_ESB hcall for interrupt management
and not loads and stores on the interrupt ESB pages.

A default -1 value is returned for the addresses of the ESB pages. The
driver ignores this condition today and performs a bogus IO mapping.
Recent changes and the DEBUG_VM configuration option make the bug

9aeaa898f5 powerpc/xive: Prevent page fault issues in the machine crash handler

When the machine crash handler is invoked, all interrupts are masked
but interrupts which have not been started yet do not have an ESB page
mapped in the Linux address space. This crashes the 'crash kexec'
sequence on sPAPR guests.

To fix, force the mapping of the ESB page when an interrupt is being
mapped in the Linux IRQ number space. This is done by setting the
initial state of the interrupt to OFF which is not necessarily the
case on PowerNV.

ea96b519b8 powerpc/xive: Fix bogus error code returned by OPAL
commit 6ccb4ac2bf8a35c694ead92f8ac5530a16e8f2c8 upstream.

There's a bug in skiboot that causes the OPAL_XIVE_ALLOCATE_IRQ call
to return the 32-bit value 0xffffffff when OPAL has run out of IRQs.
Unfortunatelty, OPAL return values are signed 64-bit entities and
errors are supposed to be negative. If that happens, the linux code
confusingly treats 0xffffffff as a valid IRQ number and panics at some
point.

A fix was recently merged in skiboot:

e97391ae2bb5 ("xive: fix return value of opal_xive_allocate_irq()")

but we need a workaround anyway to support older skiboots already
in the field.

Internally convert 0xffffffff to OPAL_RESOURCE which is the usual error
returned upon resource exhaustion.

xive_find_target_in_mask() has the following for(;;) loop which has a
bug when @first == cpumask_first(@mask) and condition 1 fails to hold
for every CPU in @mask. In this case we loop forever in the for-loop.

This is because, when @first == cpumask_first(@mask), we never hit the
condition 2 (cpu == first) since prior to this check, we would have
executed "cpu = cpumask_next(cpu, mask)" which will set the value of
@cpu to a value greater than @first or to nr_cpus_ids. When this is
coupled with the fact that condition 1 is not met, we will never exit
this loop.

This was discovered by the hard-lockup detector while running LTP test
concurrently with SMT switch tests.

To fix this, move the check for condition 2 after the check for

c3a2a87820 powerpc/mpic: Check if cpu_possible() in mpic_physmask()

In mpic_physmask() we loop over all CPUs up to 32, then get the hard
SMP processor id of that CPU.

Currently that's possibly walking off the end of the paca array, but
in a future patch we will change the paca array to be an array of
pointers, and in that case we will get a NULL for missing CPUs and
oops. eg:

ce3b0b0589 powerpc/xive: Fix trying to "push" an already active pool VP
commit b32e56e5a87a1f9243db92bc7a5df0ffb4627cfb upstream.

When setting up a CPU, we "push" (activate) a pool VP for it.

However it's an error to do so if it already has an active
pool VP.

This happens when doing soft CPU hotplug on powernv since we
don't tear down the CPU on unplug. The HW flags the error which
gets captured by the diagnostics.

Fix this by making sure to "pull" out any already active pool
first.

3b09911d3b powerpc/xive: Use hw CPU ids when configuring the CPU queues
commit 8e036c8d30a2cd9d8fc7442fbf6824e0a3e986e7 upstream.

The CPU event notification queues on sPAPR should be configured using
a hardware CPU identifier.

The problem did not show up on the Power Hypervisor because pHyp
supports 8 threads per core which keeps CPU number contiguous. This is
not the case on all sPAPR virtual machines, some use SMT=1.

Also improve error logging by adding the CPU number.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and resulted in the first patch in this series.

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

cc56939802 powerpc/xive: Clear XIVE internal structures when a CPU is removed
Commit eac1e731b5 ("powerpc/xive: guest exploitation of the XIVE
interrupt controller") introduced support for the XIVE exploitation
mode of the P9 interrupt controller on the pseries platform.

At that time, support for CPU removal was not complete on PowerVM and
CPU hot unplug remained untested. It appears that some cleanups of the
XIVE internal structures are required before releasing the CPU,
without which the kernel crashes in a RTAS call doing the CPU
isolation.

These changes fix the crash by deconfiguring the IPI interrupt source
and clearing the event queues of the CPU when it is removed.

31d1e130f4 rapidio: remove global irq spinlocks from the subsystem
Locking of config and doorbell operations should be done only if the
underlying hardware requires it.

This patch removes the global spinlocks from the rapidio subsystem and
moves them to the mport drivers (fsl_rio and tsi721), only to the
necessary places.  For example, local config space read and write
operations (lcread/lcwrite) are atomic in all existing drivers, so there
should be no need for locking, while the cread/cwrite operations which
generate maintenance transactions need to be synchronized with a lock.

Later, each driver could chose to use a per-port lock instead of a
global one, or even more granular locking.

Second, different signnnedness leads to suboptimal code in the following

MOVSXD is 1 byte longed than the same MOV.

can't be negative which can't be deduced if it is "int".

bed81ee181 powerpc/xive: introduce H_INT_ESB hcall
The H_INT_ESB hcall() is used to issue a load or store to the ESB page
instead of using the MMIO pages. This can be used as a workaround on
some HW issues. The OS knows that this hcall should be used on an
interrupt source when the ESB hcall flag is set to 1 in the hcall
H_INT_GET_SOURCE_INFO.

To maintain the frontier between the xive frontend and backend, we
introduce a new xive operation 'esb_rw' to be used in the routines
doing memory accesses on the ESBs.

eac1e731b5 powerpc/xive: guest exploitation of the XIVE interrupt controller
This is the framework for using XIVE in a PowerVM guest. The support
is very similar to the native one in a much simpler form.

Each source is associated with an Event State Buffer (ESB). This is a
two bit state machine which is used to trigger events. The bits are
named "P" (pending) and "Q" (queued) and can be controlled by MMIO.
The Guest OS registers event (or notifications) queues on which the HW
will post event data for a target to notify.

Instead of OPAL calls, a set of Hypervisors call are used to configure
the interrupt sources and the event/notification queues of the guest:

 - H_INT_GET_SOURCE_INFO

   used to obtain the address of the MMIO page of the Event State
   Buffer (PQ bits) entry associated with the source.

 - H_INT_SET_SOURCE_CONFIG

   assigns a source to a "target".

 - H_INT_GET_SOURCE_CONFIG

   determines to which "target" and "priority" is assigned to a source

 - H_INT_GET_QUEUE_INFO

   returns the address of the notification management page associated
   with the specified "target" and "priority".

 - H_INT_SET_QUEUE_CONFIG

   sets or resets the event queue for a given "target" and "priority".
   It is also used to set the notification config associated with the
   queue, only unconditional notification for the moment.  Reset is
   performed with a queue size of 0 and queueing is disabled in that
   case.

 - H_INT_GET_QUEUE_CONFIG

   returns the queue settings for a given "target" and "priority".

 - H_INT_RESET

   resets all of the partition's interrupt exploitation structures to
   their initial state, losing all configuration set via the hcalls
   H_INT_SET_SOURCE_CONFIG and H_INT_SET_QUEUE_CONFIG.

 - H_INT_SYNC

   issue a synchronisation on a source to make sure sure all
   notifications have reached their queue.

As for XICS, the XIVE interface for the guest is described in the
device tree under the "interrupt-controller" node. A couple of new
properties are specific to XIVE :

 - "reg"

   contains the base address and size of the thread interrupt
   managnement areas (TIMA), also called rings, for the User level and
   for the Guest OS level. Only the Guest OS level is taken into
   account today.

 - "ibm,xive-eq-sizes"

   the size of the event queues. One cell per size supported, contains
   log2 of size, in ascending order.

 - "ibm,xive-lisn-ranges"

   the interrupt numbers ranges assigned to the guest. These are
   allocated using a simple bitmap.

and also :

 - "/ibm,plat-res-int-priorities"

   contains a list of priorities that the hypervisor has reserved for
   its own use.

Tested with a QEMU XIVE model for pseries and with the Power hypervisor.

This happens because we're being called with our affinity mask set to
irq_default_affinity. That in turn was populated using
cpumask_setall(), which sets NR_CPUs worth of bits, not nr_cpu_ids
worth. Finally cpumask_weight() will return > nr_cpu_ids when passed a
mask which has > nr_cpu_ids bits set.

Fix it by limiting the value returned by cpumask_weight().
