a28abd7af7 memstick: core: Zero initialize id_reg in h_memstick_read_dev_id()

A new warning in clang [1] points out that id_reg is uninitialized then
passed to memstick_init_req() as a const pointer:

Commit de182cc8e8 ("drivers/memstick/core/memstick.c: avoid -Wnonnull
warning") intentionally passed this variable uninitialized to avoid an
-Wnonnull warning from a NULL value that was previously there because
id_reg is never read from the call to memstick_init_req() in
h_memstick_read_dev_id(). Just zero initialize id_reg to avoid the
warning, which is likely happening in the majority of builds using
modern compilers that support '-ftrivial-auto-var-init=zero'.

7165813568 memstick r592: make memstick_debug_get_tpc_name() static

There are no other files referencing this function, apparently
it was left global to avoid an 'unused function' warning when
the only caller is left out. With a 'W=1' build, it causes
a 'missing prototype' warning though:

drivers/memstick/host/r592.c:47:13: error: no previous prototype for 'memstick_debug_get_tpc_name' [-Werror=missing-prototypes]

Annotate the function as 'static __maybe_unused' to avoid both
problems.

b7e389235c treewide: Remove uninitialized_var() usage
commit 3f649ab728cda8038259d8f14492fe400fbab911 upstream.

Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

for x86_64, i386, arm64, arm, powerpc, powerpc64le, s390x, mips, sparc64,
alpha, and m68k.

dce890c3df memstick: r592: Fix UAF bug in r592_remove due to race condition

In r592_probe, dev->detect_timer was bound with r592_detect_timer.
In r592_irq function, the timer function will be invoked by mod_timer.

If we remove the module which will call hantro_release to make cleanup,
there may be a unfinished work. The possible sequence is as follows,
which will cause a typical UAF bug.

Fix it by canceling the work before cleanup in r592_remove.

70ae89da72 memstick: fix memory leak if card device is never registered
commit 4b6d621c9d859ff89e68cebf6178652592676013 upstream.

When calling dev_set_name() memory is allocated for the name for the
struct device.  Once that structure device is registered, or attempted
to be registerd, with the driver core, the driver core will handle
cleaning up that memory when the device is removed from the system.

Unfortunatly for the memstick code, there is an error path that causes
the struct device to never be registered, and so the memory allocated in
dev_set_name will be leaked.  Fix that leak by manually freeing it right
before the memory for the device is freed.

c2e3124dbf memstick/ms_block: Fix some incorrect memory allocation

Some functions of the bitmap API take advantage of the fact that a bitmap
is an array of long.

So, to make sure this assertion is correct, allocate bitmaps with
bitmap_zalloc() instead of kzalloc()+hand-computed number of bytes.

While at it, also use bitmap_free() instead of kfree() to keep the
semantic.

3339c3bf3d memstick: jmb38x_ms: use appropriate free function in jmb38x_ms_alloc_host()

The "msh" pointer is device managed, meaning that memstick_alloc_host()
calls device_initialize() on it.  That means that it can't be free
using kfree() but must instead be freed with memstick_free_host().

Update device_add_disk() to take an 'groups' argument so that
individual drivers can register a device with additional sysfs
attributes.

759e641acd memstick: Prevent memstick host from getting runtime suspended during card detection

We can use MEMSTICK_POWER_{ON,OFF} along with pm_runtime_{get,put}
helpers to let memstick host support runtime pm.

The rpm count may go down to zero before the memstick host powers on, so
the host can be runtime suspended.

So before doing card detection, increment the rpm count to avoid the
host gets runtime suspended. Balance the rpm count after card detection
is done.

6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

e455b69ddf misc: rtsx: Move Realtek Card Reader Driver to misc
Because Realtek card reader drivers are pcie and usb drivers,
and they bridge mmc subsystem and memstick subsystem, they are
not mfd drivers. Greg and Lee Jones had a discussion about
where to put the drivers, the result is that misc is a good
place for them, so I move all files to misc. If I don't move
them to a right place, I can't add any patch for this driver.

This converts all remaining cases of the old setup_timer() API into using
timer_setup(), where the callback argument is the structure already
holding the struct timer_list. These should have no behavioral changes,
since they just change which pointer is passed into the callback with
the same available pointers after conversion. It handles the following
examples, in addition to some other variations.

Casting from unsigned long:

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

gcc-7 produces a harmless false-postive warning about a possible NULL
pointer access:

     memcpy(mrq->data, buf, mrq->data_len);

This can't happen because the caller sets the command to 'MS_TPC_READ_REG',
which causes the data direction to be 'READ' and the NULL pointer not
accessed.

As a simple workaround for the warning, we can pass a pointer to the
data that we actually want to read into.  This is not needed here, but
also harmless, and lets the compiler know that the access is ok.

e806402130 block: split out request-only flags into a new namespace
A lot of the REQ_* flags are only used on struct requests, and only of
use to the block layer and a few drivers that dig into struct request
internals.

This patch adds a new req_flags_t rq_flags field to struct request for
them, and thus dramatically shrinks the number of common requests.  It
also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for
struct request.

Accesses to the rtsx usb device, which is the parent of the rtsx memstick
device, must not be done unless it's runtime resumed. This is currently not
the case and it could trigger various errors.

Fix this by properly deal with runtime PM in this regards. This means
making sure the device is runtime resumed, when serving requests via the
->request() callback or changing settings via the ->set_param() callbacks.

f419a08fb3 drivers/memstick/host/r592.c: avoid gcc-6 warning
The r592 driver relies on behavior of the DMA mapping API that is
normally observed but not guaranteed by the API.  Instead it uses a
runtime check to fail transfers if the API ever behaves

When CONFIG_NEED_SG_DMA_LENGTH is not set, one of the checks turns into a
comparison of a variable with itself, which gcc-6.0 now warns about:

do_div is the wrong way to divide a sector_t, as it is less efficient
when sector_t is 32-bit wide.  With the upcoming do_div optimizations,
the kernel starts warning about this:
