hvc/xen: lock console list traversal

The currently lockless access to the xen console list in
vtermno_to_xencons() is incorrect, as additions and removals from the
list can happen anytime, and as such the traversal of the list to get
the private console data for a given termno needs to happen with the
lock held. Note users that modify the list already do so with the
lock taken.
Adjust current lock takers to use the _irq{save,restore} helpers,
since the context in which vtermno_to_xencons() is called can have
interrupts disabled. Use the _irq{save,restore} set of helpers to
switch the current callers to disable interrupts in the locked region.
I haven't checked if existing users could instead use the _irq
variant, as I think it's safer to use _irq{save,restore} upfront.
While there switch from using list_for_each_entry_safe to
list_for_each_entry: the current entry cursor won't be removed as
part of the code in the loop body, so using the _safe variant is
pointless.

tty: hvc: fix return value of __setup handler

__setup() handlers should return 1 to indicate that the boot option
has been handled or 0 to indicate that it was not handled.
Add a pr_warn() message if the option value is invalid and then
always return 1.

xen/console: harden hvc_xen against event channel storms
commit fe415186b43df0db1f17fa3a46275fd92107fe71 upstream. The Xen console driver is still vulnerable for an attack via excessive number of events sent by the backend. Fix that by using a lateeoi event channel. For the normal domU initial console this requires the introduction of bind_evtchn_to_irq_lateeoi() as there is no xenbus device available at the time the event channel is bound to the irq. As the decision whether an interrupt was spurious or not requires to test for bytes having been read from the backend, move sending the event into the if statement, as sending an event without having found any bytes to be read is making no sense at all. xen/x86: make "earlyprintk=xen" work better for PVH Dom0
The xen_hvm_early_write() path better wouldn't be taken in this case; while port 0xE9 can be used, the hypercall path is quite a bit more efficient. Put that first, as it may also work for DomU-s (see also xen_raw_console_write()). While there also bail from the function when the first domU_write_console() failed - later ones aren't going to succeed. tty: don't store semi-state into tty drivers
When a tty driver pointer is used as a return value of struct console's device() hook, don't store a semi-state into global variable which holds the tty driver. It could mean console::device() would return a bogus value. This is important esp. after the next patch where we switch from alloc_tty_driver to tty_alloc_driver. tty_alloc_driver returns ERR_PTR in case of error and that might have unexpected results as the code doesn't expect this. hvsi: don't panic on tty_register_driver failure
The alloc_tty_driver failure is handled gracefully in hvsi_init. But tty_register_driver is not. panic is called if that one fails. So handle the failure of tty_register_driver gracefully too. This will keep at least the console functional as it was enabled earlier by console_initcall in hvsi_console_init. Instead of shooting down the whole system. This means, we disable interrupts and restore hvsi_wait back to poll_for_state(). hvterm_raw_put_chars() calls hvc_put_chars(), which may return -EAGAIN when the underlying hcall returns a "busy" status, but udbg_hvc_putc() doesn't handle this. When using xmon on a PowerVM guest, this can result in incomplete or garbled output when printing relatively large amounts of data quickly, such as when dumping the kernel log buffer. Call again on -EAGAIN. vio: make remove callback return void
The driver core ignores the return value of struct bus_type::remove() because there is only little that can be done. To simplify the quest to make this function return void, let struct vio_driver::remove() return void, too. All users already unconditionally return 0, this commit makes it obvious that returning an error code is a bad idea. Note there are two nominally different implementations for a vio bus: one in arch/sparc/kernel/vio.c and the other in arch/powerpc/platforms/pseries/vio.c. This patch only adapts the powerpc one. Before this patch for a device that was bound to a driver without a remove callback vio_cmo_bus_remove(viodev) wasn't called. As the device core still considers the device unbound after vio_bus_remove() returns calling this unconditionally is the consistent behaviour which is implemented here. If hvcs_probe() succeeded dev_set_drvdata() is called with a non-NULL value, and if hvcs_probe() failed hvcs_remove() isn't called. So there is no way dev_get_drvdata() can return NULL in hvcs_remove() and the check can just go away. Some debuggers, such as Trace32 from Lauterbach GmbH, do not handle reads/writes from/to DCC on secondary cores. Each core has its own DCC device registers, so when a core reads or writes from/to DCC, it only accesses its own DCC device. Since kernel code can run on any core, every time the kernel wants to write to the console, it might write to a different DCC. In SMP mode, Trace32 only uses the DCC on core 0. In AMP mode, it creates multiple windows, and each window shows the DCC output only from that core's DCC. The result is that console output is either lost or scattered across windows. Selecting this option will enable code that serializes all console input and output to core 0. The DCC driver will create input and output FIFOs that all cores will use. Reads and writes from/to DCC are handled by a workqueue that runs only core 0. tty: hvcs: Don't NULL tty->driver_data until hvcs_cleanup()
The code currently NULLs tty->driver_data in hvcs_close() with the
intent of informing the next call to hvcs_open() that device needs to be
reconfigured. However, when hvcs_cleanup() is called we copy hvcsd from
tty->driver_data which was previoulsy NULLed by hvcs_close() and our
call to tty_port_put(&hvcsd->port) doesn't actually do anything since
&hvcsd->port ends up translating to NULL by chance. This has the side
effect that when hvcs_remove() is called we have one too many port
references preventing hvcs_destuct_port() from ever being called. This
also prevents us from reusing the /dev/hvcsX node in a future
hvcs_probe() and we can eventually run out of /dev/hvcsX devices.
Fix this by waiting to NULL tty->driver_data in hvcs_cleanup().

tty: hvc_console, fix crashes on parallel open/close
hvc_open sets tty->driver_data to NULL when open fails at some point. Typically, the failure happens in hp->ops->notifier_add(). If there is a racing process which tries to open such mangled tty, which was not closed yet, the process will crash in hvc_open as tty->driver_data is NULL. All this happens because close wants to know whether open failed or not. But ->open should not NULL this and other tty fields for ->close to be happy. ->open should call tty_port_set_initialized(true) and close should check by tty_port_initialized() instead. So do this properly in this driver. So this patch removes these from ->open: * tty_port_tty_set(&hp->port, NULL). This happens on last close. * tty->driver_data = NULL. Dtto. * tty_port_put(&hp->port). This happens in shutdown and until now, this must have been causing a reference underflow, if I am not missing something.
