4d69cdba2c SUNRPC: ensure the matching upcall is in-flight upon downcall

Commit 9130b8dbc6 ("SUNRPC: allow for upcalls for the same uid
but different gss service") introduced `auth` argument to
__gss_find_upcall(), but in gss_pipe_downcall() it was left as NULL
since it (and auth->service) was not (yet) determined.

When multiple upcalls with the same uid and different service are
ongoing, it could happen that __gss_find_upcall(), which returns the
first match found in the pipe->in_downcall list, could not find the
correct gss_msg corresponding to the downcall we are looking for.
Moreover, it might return a msg which is not sent to rpc.gssd yet.

We could see mount.nfs process hung in D state with multiple mount.nfs
are executed in parallel.  The call trace below is of CentOS 7.9
kernel-3.10.0-1160.24.1.el7.x86_64 but we observed the same hang w/
elrepo kernel-ml-6.0.7-1.el7.

The scenario is like this. Let's say there are two upcalls for
services A and B, A -> B in pipe->in_downcall, B -> A in pipe->pipe.

When rpc.gssd reads pipe to get the upcall msg corresponding to
service B from pipe->pipe and then writes the response, in
gss_pipe_downcall the msg corresponding to service A will be picked
because only uid is used to find the msg and it is before the one for
B in pipe->in_downcall.  And the process waiting for the msg
corresponding to service A will be woken up.

Actual scheduing of that process might be after rpc.gssd processes the
next msg.  In rpc_pipe_generic_upcall it clears msg->errno (for A).
The process is scheduled to see gss_msg->ctx == NULL and
gss_msg->msg.errno == 0, therefore it cannot break the loop in
gss_create_upcall and is never woken up after that.

This patch adds a simple check to ensure that a msg which is not
sent to rpc.gssd yet is not chosen as the matching upcall upon
receiving a downcall.

cedaf73c8b SUNRPC: Fix svcxdr_init_encode's buflen calculation

Commit 2825a7f907 ("nfsd4: allow encoding across page boundaries")
added an explicit computation of the remaining length in the rq_res
XDR buffer.

The computation appears to suffer from an "off-by-one" bug. Because
buflen is too large by one page, XDR encoding can run off the end of
the send buffer by eventually trying to use the struct page address
in rq_page_end, which always contains NULL.

6b55707ff8 SUNRPC: Fix svcxdr_init_decode's end-of-buffer calculation

Ensure that stream-based argument decoding can't go past the actual
end of the receive buffer. xdr_init_decode's calculation of the
value of xdr->end over-estimates the end of the buffer because the
Linux kernel RPC server code does not remove the size of the RPC
header from rqstp->rq_arg before calling the upper layer's
dispatcher.

The server-side still uses the svc_getnl() macros to decode the
RPC call header. These macros reduce the length of the head iov
but do not update the total length of the message in the buffer
(buf->len).

A proper fix for this would be to replace the use of svc_getnl() and
friends in the RPC header decoder, but that would be a large and
invasive change that would be difficult to backport.

d3c262f584 SUNRPC: Fix xdr_encode_bool()
commit c770f31d8f580ed4b965c64f924ec1cc50e41734 upstream.

I discovered that xdr_encode_bool() was returning the same address
that was passed in the @p parameter. The documenting comment states
that the intent is to return the address of the next buffer
location, just like the other "xdr_encode_*" helpers.

The result was the encoded results of NFSv3 PATHCONF operations were
not formed correctly.

f5e13d700a SUNRPC: Fix NFSD's request deferral on RDMA transports
commit 773f91b2cf3f52df0d7508fdbf60f37567cdaee4 upstream.

Trond Myklebust reports an NFSD crash in svc_rdma_sendto(). Further
investigation shows that the crash occurred while NFSD was handling
a deferred request.

This patch addresses two inter-related issues that prevent request
deferral from working correctly for RPC/RDMA requests:

1. Prevent the crash by ensuring that the original
   svc_rqst::rq_xprt_ctxt value is available when the request is
   revisited. Otherwise svc_rdma_sendto() does not have a Receive
   context available with which to construct its reply.

2. Possibly since before commit 71641d99ce ("svcrdma: Properly
   compute .len and .buflen for received RPC Calls"),
   svc_rdma_recvfrom() did not include the transport header in the
   returned xdr_buf. There should have been no need for svc_defer()
   and friends to save and restore that header, as of that commit.
   This issue is addressed in a backport-friendly way by simply
   having svc_rdma_recvfrom() set rq_xprt_hlen to zero
   unconditionally, just as svc_tcp_recvfrom() does. This enables
   svc_deferred_recv() to correctly reconstruct an RPC message
   received via RPC/RDMA.

5d69622eb0 SUNRPC: Don't call connect() more than once on a TCP socket
commit 89f42494f92f448747bd8a7ab1ae8b5d5520577d upstream.

Avoid socket state races due to repeated calls to ->connect() using the
same socket. If connect() returns 0 due to the connection having
completed, but we are in fact in a closing state, then we may leave the
XPRT_CONNECTING flag set on the transport.

2de88544b3 NFSD: Have legacy NFSD WRITE decoders use xdr_stream_subsegment()

Refactor.

Now that the NFSv2 and NFSv3 XDR decoders have been converted to
use xdr_streams, the WRITE decoder functions can use
xdr_stream_subsegment() to extract the WRITE payload into its own
xdr_buf, just as the NFSv4 WRITE XDR decoder currently does.

That makes it possible to pass the first kvec, pages array + length,
page_base, and total payload length via a single function parameter.

The payload's page_base is not yet assigned or used, but will be in
subsequent patches.

dc48e0abee SUNRPC enforce creation of no more than max_connect xprts
If we are adding new transports via rpc_clnt_test_and_add_xprt()
then check if we've reached the limit. Currently only pnfs path
adds transports via that function but this is done in
preparation when the client would add new transports when
session trunking is detected. A warning is logged if the
limit is reached.

3a3f976639 SUNRPC keep track of number of transports to unique addresses
Currently, xprt_switch keeps a number of all xprts (xps_nxprts)
that were added to the switch regardless of whethere it's an
nconnect transport or a transport to a trunkable address.
Introduce a new counter to keep track of transports to unique
destination addresses per xprt_switch.

a4ae308143 SUNRPC: Move client-side disconnect injection
Disconnect injection stress-tests the ability for both client and
server implementations to behave resiliently in the face of network
instability.

Convert the existing client-side disconnect injection infrastructure
to use the kernel's generic error injection facility. The generic
facility has a richer set of injection criteria.

07a92d009f svcrdma: Convert rdma->sc_rw_ctxts to llist
Relieve contention on sc_rw_ctxt_lock by converting rdma->sc_rw_ctxts
to an llist.

The goal is to reduce the average overhead of Send completions,
because a transport's completion handlers are single-threaded on
one CPU core. This change reduces CPU utilization of each Send
completion by 2-3% on my server.

b6c2bfea09 svcrdma: Relieve contention on sc_send_lock.
/proc/lock_stat indicates the the sc_send_lock is heavily
contended when the server is under load from a single client.

To address this, convert the send_ctxt free list to an llist.
Returning an item to the send_ctxt cache is now waitless, which
reduces the instruction path length in the single-threaded Send
handler (svc_rdma_wc_send).

The goal is to enable the ib_comp_wq worker to handle a higher
RPC/RDMA Send completion rate given the same CPU resources. This
change reduces CPU utilization of Send completion by 2-3% on my
server.

6c8c84f525 svcrdma: Fewer calls to wake_up() in Send completion handler
Because wake_up() takes an IRQ-safe lock, it can be expensive,
especially to call inside of a single-threaded completion handler.
What's more, the Send wait queue almost never has waiters, so
most of the time, this is an expensive no-op.

As always, the goal is to reduce the average overhead of each
completion, because a transport's completion handlers are single-
threaded on one CPU core. This change reduces CPU utilization of
the Send completion thread by 2-3% on my server.

2f0f88f42f SUNRPC: Add svc_rqst_replace_page() API
Replacing a page in rq_pages[] requires a get_page(), which is a
bus-locked operation, and a put_page(), which can be even more
costly.

To reduce the cost of replacing a page in rq_pages[], batch the
put_page() operations by collecting "freed" pages in a pagevec,
and then release those pages when the pagevec is full. This
pagevec is also emptied when each RPC completes.

438623a06b SUNRPC: Add svc_rqst::rq_auth_stat
I'd like to take commit 4532608d71 ("SUNRPC: Clean up generic
dispatcher code") even further by using only private local SVC
dispatchers for all kernel RPC services. This change would enable
the removal of the logic that switches between
svc_generic_dispatch() and a service's private dispatcher, and
simplify the invocation of the service's pc_release method
so that humans can visually verify that it is always invoked
properly.

All that will come later.

First, let's provide a better way to return authentication errors
from SVC dispatcher functions. Instead of overloading the dispatch
method's *statp argument, add a field to struct svc_rqst that can
hold an error value.

6f081693e7 sunrpc: remove an offlined xprt using sysfs
Once a transport has been put offline, this transport can be also
removed from the list of transports. Any tasks that have been stuck
on this transport would find the next available active transport
and be re-tried. This transport would be removed from the xprt_switch
list and freed.

5b7eb78486 SUNRPC: take a xprt offline using sysfs
Using sysfs's xprt_state attribute, mark a particular transport offline.
It will not be picked during the round-robin selection. It's not allowed
to take the main (1st created transport associated with the rpc_client)
offline. Also bring a transport back online via sysfs by writing "online"
and that would allow for this transport to be picked during the round-
robin selection.

d408ebe04a sunrpc: add add sysfs directory per xprt under each xprt_switch
Add individual transport directories under each transport switch
group. For instance, for each nconnect=X connections there will be
a transport directory. Naming conventions also identifies transport
type -- xprt-<id>-<type> where type is udp, tcp, rdma, local, bc.

4c52729377 kernel.h: split out kstrtox() and simple_strtox() to a separate header
kernel.h is being used as a dump for all kinds of stuff for a long time.
Here is the attempt to start cleaning it up by splitting out kstrtox() and
simple_strtox() helpers.

At the same time convert users in header and lib folders to use new
header.  Though for time being include new header back to kernel.h to
avoid twisted indirected includes for existing users.

2a1e4f21d8 svcrdma: Normalize Send page handling
Currently svc_rdma_sendto() migrates xdr_buf pages into a separate
page list and NULLs out a bunch of entries in rq_pages while the
pages are under I/O. The Send completion handler then frees those
pages later.

Instead, let's wait for the Send completion, then handle page
releasing in the nfsd thread. I'd like to avoid the cost of 250+
put_page() calls in the Send completion handler, which is single-
threaded.

e844d307d4 svcrdma: Add a "deferred close" helper
Refactor a bit of commonly used logic so that every site that wants
a close deferred to an nfsd thread does all the right things
(set_bit(XPT_CLOSE) then enqueue).

Also, once XPT_CLOSE is set on a transport, it is never cleared. If
XPT_CLOSE is already set, then the close is already being handled
and the enqueue can be skipped.

bddfdbcddb NFSD: Extract the svcxdr_init_encode() helper
NFSD initializes an encode xdr_stream only after the RPC layer has
already inserted the RPC Reply header. Thus it behaves differently
than xdr_init_encode does, which assumes the passed-in xdr_buf is
entirely devoid of content.

nfs4proc.c has this server-side stream initialization helper, but
it is visible only to the NFSv4 code. Move this helper to a place
that can be accessed by NFSv2 and NFSv3 server XDR functions.
